چکیده

Since the invention of the first implantable shunt valve by Nulsen and Spitz (12) almost 50 years ago, there has been a remarkable number of ingenious modifications and new designs of shunt equipment to treat pediatric hydrocephalus. These developments were in response to the immediately evident high shunt failure rate. These designs included antisiphon devices (15), on-off devices, gravity-actuated changes in opening pressure, and even externally adjustable valves, some with electromagnetic programmers (16) (Fig. 18.1). Aside from the introduction of the silicone elastomer material, there has in fact been little clinical impact on the treatment with patients with shunts. Moreover, there has been a recognition of the unexpected complications-cor pulmonale (11) and shunt nephritis (19) from cardiac shunts, bowel erosion from springcoiled catheters (1), obstruction of anti-siphon devices by capsule formation (3), and tonsilar herniation by lumboperitoneal shunts (2). In fact, each new shunt equipment design has in many cases brought along its own unique set of complications. Interpretation of the results of new shunt hardware was often hampered by the design of the studies. They often contained small retrospective series of patients, loosely defined inclusion criteria, poor definition of outcome events, short follow-up, and inappropriate statistical analysis. These studies were often conducted by enthusiasts of the devices who had vested interests in the outcome, including financial incentives. Early enthusiastic reports were often followed by less enthusiastic ones, often about unexpected complications.

A Giant Abdominal Cerebrospinal Fluid Pseudocyst

Background and Aim: Ventriculoperitoneal shunt (VPS) is the most commonly used procedure for treatment of hydrocephalus in children. Abdominal cerebrospinal fluid pseudocyst is a rare complication but potentially fatal. The pathogenesis of this complication remains unclear. This complication should be considered during differential diagnosis of an abdominal mass. Our aim is to report how large ...

Micro devices for a cerebrospinal fluid (CSF) shunt system

This paper reports a novel cerebrospinal fluid (CSF) shunt system for the hydrocephalus patients. The CSF shunt system consists of a micro telemetry pressure sensor, an electromagnetic micropump and a controller. The pressure sensor has a flexible p+ diaphragm and a planar copper coil that construct an LC resonant circuit. The cerebrospinal pressure is measured from the phase shift at the reson...

Evaluation of Microbial Bacterial and Fungal Diversity in Cerebrospinal Fluid Shunt Infection

BACKGROUND Cerebrospinal fluid shunt infection can be recalcitrant. Recurrence is common despite appropriate therapy for the pathogens identified by culture. Improved diagnostic and therapeutic approaches are required, and culture-independent molecular approaches to cerebrospinal fluid shunt infections have not been described. OBJECTIVES To identify the bacteria and fungi present in cerebrosp...

Investigation of Cerebrospinal Fluid Flow Characteristics Through Adult and Pediatric Shunt Tubing

Shunt malfunction in adult and pediatric populations continues to be a perplexing problem. We examined artificial cerebrospinal fluid flow differential between adult and pediatric shunt systems. Flow through adult and pediatric-sized catheters was compared with artificial CSF using shunt systems containing new catheters, explanted catheters, 2-piece catheters, 3-piece catheters and with varying...

Abdominal cerebrospinal fl uid pseudocysts in patients with ventriculoperitoneal shunts: 30 years of experience*

Aim: We evaluated the treatment outcome of the patients having cerebrospinal fluid pseudocyst following ventriculo-peritoneal shunt. Materials and Methods: During the period of 1975 to 2005, 392 hydrocephalic patients underwent ventriculo-peritoneal shunt, of these eight developed abdominal cerebrospinal fluid pseudocyst. The medical records regarding the etiology of hydrocephalus, age of shunt...

Ascites as first presentation of ventriculo-peritoneal shunt infection in a neonate.

Infection is a common complication of ventriculoperitoneal shunts, and ascites is one of the rare manifestations of shunt infection. We report a neonate in whom shunt infection is presented only by ascites. The causative organism, coagulase negative staphylococci, was detected only in the cerebrospinal fluid although peritoneal fluid analysis was consistent with infection. Our patient shows the...
